RELEASE OF LIABILITY*	Football Proper urges all participants to obtain a physical examination from a doctor prior to exercise. By submitting this form I confirm that I have read this Release of Liability and that I agree that as the parent / guardian, I submit that the participant is in good health and I give my permission for him/her to participate in a vigorous athletic training program. Player has no previous sickness, illness, disease or bodily injury that is contradictory to participation. I understand that Football Proper is not responsible for any physical injury that may occur during the course of training activities. I hereby release, hold harmless, discharge and agree not to sue Football Proper, its directors, officers, coaches, employees, volunteers, agents, sponsors, advertisers, owners/leasers of the premises for all liability from player participation in these and any other related travel, lodging, social/recreational activities. I also understand Football Proper retains the right to use for publicity and advertising, photographs and videos taken of participants. In the event that player is injured, I give my permission for such medical procedures as may be deemed necessary. I have read Football ProperI's Release of Liability and Payment/Cancellation Policy.
Payment and Cancellation Policy*	By checking this box and submitting this form, I confirm that I have read and understand the following Payment and Cancellation Policy. Football Proper Summer Camp fees are due at the time of registration by either cash, check or credit card. Participants who wish to withdraw due to injury, dissatisfaction, scheduling, or any other reason may do so at any time through written cancellation but are not guaranteed full refund.
